Home health services and hospice care is located on the far west side of Gottlieb Memorial Hospital, near the Emergency Department, on North Avenue near Fifth Avenue.


Loyola's home health nurses give patients the care and support needed to return to independent daily living after a surgery or extended illness. They are trained in:

  • Education and monitoring of your health

  • Nutritional assessment and education
  • Rehabilitation services

Hospice care professionals compassionately care for terminally ill patients in their homes or in the hospital. They provide:
